Conditions and Forms

I'm having a problem with a condition working. I've used the same formula on a different screen and all the forms need to be filled in before it allows you to go to the next page. However, when I've copied the conditions to this screen when I click next, it goes to the next page. Even when the form boxes are empty? I can't see what I'm doing wrong.
