How to Clear Form

Hi ,

Now I am referring to

After I input data into data list through form. The form fields are not getting cleared. Any help will be highly appreciated. Thanks in advance

You can add a 'Set value action' on the button and choose Value is.. Fixed, and leave the text field empty for each of the form fields.
